The following forms must be submitted throughout the course of the Minor:
Declaring and Completing the Minor:
Minor Declaration of Intent Form
Must be submitted to indicate a student’s intention to declare the Minor.
Minor Completion Form
Must be signed by the student’s Major Advisor and then submitted to a GPP Advisor before they graduate.
Practice Summary Form
Indicates initial details about the practice experience the student has secured. This form must be submitted to a GPP Advisor to receive a Course Entry Code to enroll in GPP 105, the second core course for the Minor.
Practice Agreement Form
Indicates final details of the practice experience. This form must be signed by both the student and the student’s supervisor at the practice experience organization. This form must be submitted at least six weeks prior to beginning the practice experience.
Must be submitted before beginning the Practice Experience. This form will be given to students during the Pre-Departure Orientation. Students who are not able to attend the orientation can download the form from the GPP Minor bcourses site.
This form asks students to document key information (e.g. emergency contacts) that will be helpful during practice experiences outside of the Bay Area. Students must keep a copy for themselves and submit a copy to the GPP Advising Office at least six weeks prior to beginning the practice experience.
Course Substitution Form
Submit this form to request approval to take an elective course which is not listed on the approved elective course lists.
Thesis Approval Form
Submit this form to request approval to use a major thesis as your reflection requirement for the Minor.
