Important Forms

The forms below must be submitted throughout the course of the Minor. 
Any form that is in pdf format may be sumbitted using this link.

Declaring and Completing the Minor

Minor Declaration of Intent Form
Must be submitted to indicate a student’s intention to declare the Minor.

Minor Completion Form
Must be signed by the student’s Major Advisor and then submitted to a GPP Advisor before they graduate.

Practice Experience

Practice Approval Form
Indicates initial details about the practice experience the student has secured. This form must be submitted to a GPP Advisor to receive a Course Entry Code to enroll in GPP 105, the second core course for the Minor.

Practice Agreement Form
Indicates final details of the practice experience. This form must be signed by both the student and the student’s supervisor at the practice experience organization. This form must be submitted at least six weeks prior to beginning the practice experience.

Liability Waiver
Must be submitted before beginning the Practice Experience.

Travel Profile
This form asks students to document key information (e.g. emergency contacts) that will be helpful during practice experiences outside of the Bay Area. Students must keep a copy for themselves and submit a copy to the GPP Advising Office at least six weeks prior to beginning the practice experience.

Courses

Course Substitution Form
Submit this form to request approval to take an elective course which is not listed on the approved elective course lists.

Thesis Approval Form
Submit this form to request approval to use a major thesis as your reflection requirement for the Minor.

Host and Fellow Responsibilities

Host Organizations

  • Identify staff supervisor to manage I&E Climate Action Fellow
  • Submit fellowship description and tasks
  • Engage in the matching process
  • Mentor and advise students
  • Communicate with Berkeley program director and give feedback on the program.

Berkeley Program Director​

  • Communicate with host organizations, students, and other university departments to ensure smooth program operations

Student Fellows

  • Complete application and cohort activities
  • Communicate with staff and host organizations
  • Successfully complete assignments from host organization during summer practicum
  • Summarize and report summer experience activities post-fellowship
